I would start with something like a program called perdition

The problem with >65536 users is file ownership. You can have more than
one login assiciated with a UID but how do you prevent one user from
reading another's email? Better off using multiple systems and perdition
is designed to be a gateway for such a setup where it redirects the user
to the mail server that handles their account without the user having to
know which server that is.
